1. What the guarantee covers

If you are an eligible patient who completes 6 consecutive months in an Arizona Trim Clinic semaglutide or tirzepatide weight-loss program and you do not lose at least 15 pounds (lbs), Arizona Trim Clinic will refund the eligible program fees you paid for that 6-month period, as described below.

2. Eligibility requirements

To qualify, all of the following must be true:

1. Continuous 6-month enrollment. You were enrolled in a semaglutide or tirzepatide program for 6 consecutive months, with no gaps, pauses, or missed payments.
2. Prescribed dosing followed. You took your medication as prescribed and followed the dosing schedule set by your provider, including dose adjustments. Missing doses, stopping, or changing your dose without provider approval makes you ineligible.
3. Monthly check-ins attended. You attended every scheduled monthly check-in. A check-in rescheduled with at least 24 hours’ notice and completed within 7 days counts as attended.
4. Clinic-recorded weigh-ins. Your starting weight and every monthly weight were measured and recorded on the clinic’s scale by our staff. Home scale readings, photos, or self-reported weights are not accepted.
5. Starting weight on record. Your starting weight was recorded at the clinic at or before your first dose in the program.
6. Medically eligible throughout. Your provider did not pause or stop your medication for safety reasons caused by not following medical instructions.
7. Account in good standing. All program fees for the 6-month period are paid in full.

3. How weight loss is measured

Weight loss equals your clinic-recorded starting weight minus your clinic-recorded weight at your final check-in, held between 26 and 28 weeks after your first dose. Weigh-ins should be done under similar conditions (for example, similar time of day and clothing). Clinic records are final.

4. How to claim a refund

1. Submit a written refund request to Trangiabao880@gmail.com or in person at the clinic within 30 days after your final (6-month) check-in. Please do not include medical details in your email; we will review your records internally.
2. Our team will review your enrollment, payment, attendance, dosing, and weigh-in records.
3. We will respond in writing within 14 business days.
4. Approved refunds are issued to the original payment method within 30 days of approval.

5. What is refundable

• Refundable: program fees you paid directly to Arizona Trim Clinic for the semaglutide or tirzepatide program during the 6-month guarantee period.
• Not refundable: laboratory fees, third-party pharmacy charges, other services (such as cryolipolysis, injections, vitamin or wellness services, hair loss treatment, or phentermine), products, and any amounts paid by insurance, discounts, or promotional credits. The value of any free month is not refundable.

6. Other conditions

• One guarantee per patient. The guarantee applies to your first 6-month program only.
• Patients who are not medically eligible for GLP-1 medication are not enrolled and are not covered.
• Your health and safety come first. If your provider changes or stops treatment for medical reasons not caused by non-adherence, our team will review your eligibility case by case.
• Arizona Trim Clinic may change or end this guarantee for future enrollments at any time. Changes do not affect patients already enrolled under these terms.
• This guarantee does not promise any specific result. Individual results vary.
